(Baltimore, MD) -- Maryland hospitals are reporting a spike in medical errors resulting in serious injuries and deaths.
A new report from the state health department's Office of Health Care Quality shows there were 832 "adverse events" in fiscal year 2022, which is the most since record-keeping began in 2004.
Of those incidents, 769 were defined as "Level One" events, meaning the medical blunder caused a patient's death or serious injury.
In one case, three people died after a maintenance worker accidentally shut off an unlabeled oxygen line.
